Brief:

We have a client who is from BD13 and who requires an examination with a Eye Specialist in relation to a medico legal matter. Our client is 39 years old and has been referred to a Eye Specialist due to effect in the eye sight following a Road Traffic Accident. 

Notes: 11 Pages of optician records + 24 Pages of GP records
