Job Description
We are looking for an experienced Care Coordinator to join our team and support the development of a bespoke Community Care/Support Service across Buckinghamshire.
The role involves coordinating individualized care services for clients, enabling them to remain in their homes and communities.
Role: Care Coordinator
Location: Amersham, Buckinghamshire
Job Type: Full-time, Permanent
Salary: £24,000–£25,000 per year
What you will do:
- Ensure compliance with policies, procedures, and regulations.
- Conduct client assessments, reviews, and update care plans as needed.
- Liaise with families, medical professionals, and stakeholders for comprehensive support.
- Schedule carers, manage staff hours, and update recruitment files.
- Conduct staff supervisions, spot checks, and appraisals.
- Provide on-call cover and address punctuality or continuity issues.
- Promote equality, diversity, and inclusion.
- Deliver care in the community and transport staff if required.
- Identify and implement service improvements.
What you will have:
- At least 3 years of experience in providing care (required).
- NVQ Level 3 Health and Social Care
- A valid UK Driving Licence (required).
- Excellent knowledge of the Buckinghamshire area.
- Willingness to travel (up to 75%).
