Job Description

We are seeking a Senior Social Work Practitioner to join our team in Illford. You will provide legal and procedural advice and services in the most effective and efficient manner to meet the requirements of the Council, Cabinet, and other member bodies, services, and departments.

Duration: Ongoing
Hours per Week: 36 minimum
Rate: £28.09  (PAYE), £35.58 (Ltd)

Key Responsibilities:

• Ensure that patients admitted to hospital are supported with services that meet their community needs or recommended pathways.
• Demonstrate knowledge of the discharge to assess model of care.
• Undertake all duties as per the job title and ensure compliance with statutory responsibilities in Adult Social Care.
• Supervise and manage the performance of social workers, providing guidance and support as needed.
• Maintain an up-to-date understanding of the range of legislation that underpins statutory responsibilities in Adult Social Care.
• Deputise for the Hospital Social Work Manager when required, ensuring continuity of service and leadership.

Requirements:

• HCPC Registration
• 3 years of work history (1 year must be in relevant feild)
• Eligibility to work in the UK
• Enhanced DBS on the Update Service
