Job Description

We are seeking a Qualified Social Worker to join our Adults Social Care team. The primary responsibility will be to work on the the discharge to practise model,

coordinating services for the recommended discharge pathway to meet the assessed needs of patients prior to discharge from the hospital.

Duration: Ongoing
Hours per Week: 36 minimum
Rate: £22.47 (PAYE), £28.33 (Ltd)

Key Responsibilities:

• Arrange services for the recommended discharge pathway to meet the assessed needs of patients prior to discharge from hospital.
• Undertake reviews of patient needs and care plans.
• Ensure knowledge of relevant legislation required to implement the responsibilities of local authorities.
• Conduct assessments as required.
• Collaborate with healthcare professionals and other stakeholders to facilitate smooth transitions for patients.

Requirements:

• HCPC Registration
• 3 years of work history (1 year must be in relevant feild).
• Eligibility to work in the UK.
• Enhanced DBS on the Update Service
