Job Description

We are currently seeking 5 experienced Qualified Social Workers to join our dynamic team in Sutton.
Our service operates within an integrated framework, collaborating closely with Adult Social Care and NHS services to support adults and older people in the community. You will play a crucial role in our Urgent Response service, aimed at preventing hospital admissions, as well as providing vital Rehab services.

Requirements:
  • 2+ years post-qualifying experience in the UK social care sector.
  • Preferably experienced in physical disabilities, hospital discharge, or community neuro settings.
  • Knowledge of relevant legislation (Care Act, Mental Capacity Act).
  • Familiarity with MOSAIC or similar case management systems.
  • Willingness to conduct essential face-to-face visits with provided PPE.
  • Enhanced DBS check and Social Work England registration required.

Responsibilities:
  • Conduct comprehensive assessments of service users' needs, ensuring a holistic understanding of their circumstances.
  • Conduct home visits to assess service users' living conditions and suitability for support services.
  • Implement and oversee safeguarding measures to ensure the welfare and safety of vulnerable adults.
  • Conduct Carers Assessments to identify the support needs of individuals caring for service users.
  • Provide coverage for duty shifts as required, responding promptly to urgent situations.
  • Perform Mental Capacity Assessments to determine individuals' ability to make decisions about their care.
  • Actively participate in Multi-Disciplinary Meetings (MDM), contributing valuable insights and coordinating care plans.
